EPIC Dental

Like our other products, the EPIC Dental Plan is a group insurance plan that offers flexibility in deductible, coinsurance, and benefit design, allowing you to give your employees the benefits they need while staying within your budget. We have many options to design a plan that best fits your needs. Here's how the funding of each plan works.

Traditional Plan -- This plan can be purchased where the employer pays 100% of the premium, or can be payed partly by the employer and partly by the employees. The Traditional Plan covers Preventive, Basic, and Major services. Depending on group size, orthodontia is optional.

Base Plan with Buy-up Option -- here, you (or you and your employee) pay a base premium which covers Preventive and Basic Dental coverage. Eligible employees then have the option to purchase -- or "buy up" to -- Major or Major with Orthodontia coverage on an individual basis depending on the option you choose to offer. You will remit the premium along with the Base Plan. You can also purchase the Base Plan on a stand-alone bases.

With other EPIC coverage: Minimum two lives
Stand alone: Minimum ten lives

Free program for all groups with EPIC Dental or Vision Insurance.

In an effort to provide our groups with the best value in benefit dollars, EPIC is pleased to announce our new EyeMed vision discount program.

EyeMed Vision CareEyeMed offers substantial savings on eye care, eyewear, and laser vision correction procedures.  To receive your discount, simply show your EPIC member ID card when you visit one of EyeMed’s 16,000 participating providers. 

EyeMed’s provider network includes many familiar optical retailers, such as: LensCrafters, Pearle Vision, Sears Optical, Shopko Eye Care Center, Target Optical, JCPenny Optical, and many more.  You will also have a choice of many other local, private providers.
