Voluntary Dental

Voluntary products allow employees to purchase group-based insurance plans on an individual basis. Eligible employees who choose to participate in the Voluntary Dental plan pay 100% of the premium. Your role is simple -- select a benefit design to offer your employees, provide administrative support, and collect premiums from employees through easy payroll deduction.

 

Group Size Requirements for EPIC Dental Plans
With other EPIC coverage: Minimum two lives
Stand alone: Minimum ten lives

The Affinity Program provides significant member savings on professional vision care services and eyewear. This program is not an insurance plan – it offers fixed out-of-pocket costs and discounts.

To receive your discount, simply use your member ID card when you visit one of Davis Vision’s participating providers.
