Dependent Term Life
This coverage conveniently extends your benefits package to include life coverage for eligible spouses and dependent children.
General information regarding EPIC Dependent Term Life coverage:
- A spouse may not be insured as a dependent if he or she is also covered under your group's EPIC Base Term Life coverage.
- Dependent children may not be insured by both a husband and wife who work for the same employer.
- An employee electing dependent life coverage must be covered under the EPIC Base Life coverage.
- If dependent life is offered on a contributory basis (i.e., employees contribute to the premium cost), 75% of all eligible employees must participate in the plan.
- Dependents are eligible to convert to an individual life insurance policy without health underwriting, if they apply within 31 days from the date their EPIC Dependent Term Life insurance ends.
Choose from the following benefit levels:
